Under the hold if you are using gaffers tape, next to if you are using duck tape (it's impossible to get off later if you put it under)

I think it's always a good idea to try and get some of the tape under the hold because it helps prevent the tape from being accidentally pulled off the wall by climbers.

Under the hold, but as mentioned above: duct tape can be hard to clean once compressed under a hold. For this reason just put a little under the hold.
It's also a good idea to brush the tape after application. I use a small brass-bristled (toothbrush-sized)brush. Brush along the length of the tape and it will help it stick much better, and last much longer. Be careful not to catch and lift the corners of the tape.

We put the (duct style)tape next to the hold coming off at 45 degree angles and only 4'' long due to the lack of wall space we available.

Under the hold...paying attention to how people will be using their feet for the hold. I make sure the tape is visible from above but not in postion where sloppy footwork will tear it off.
I pound on the tape with my index and middle finger (making sure the texture shows through the tape). This technique takes some conditioning to toughen up your tips, but I like it more then using a brush or a wrench handle.
The only reason I don't like to put the tape under the hold is pure laziness. Putting the tape under the hold requires A) taping while you set, hoping that it will all work out on your first placement...or B) Loosening EVERY hold after you set to apply the tape. I'd rather replace tape as needed (not very often if you place the tape in a good spot).
Of course there is always the super anal way...Put the tape under the hold with a 1/2 inch folded over for an easy way to pull the tape when it is time to strip.
